What is Anorchia ?

Anorchia, also known as “vanishing testes syndrome” or congenital absence of the testes, is a very rare condition in which a person with a male (XY) karyotype is born without testes. This condition is distinct from undescended testes (cryptorchidism), where the testes are present but have not moved into the scrotum.

There are two main types of anorchia

1)Congenital Anorchia: This is the most common form and occurs when the testes fail to develop or disappear during fetal development.

2)Acquired Anorchia: This is the absence of testicles due to surgical removal, trauma, or infection.

​A) Congenital Anorchia (Vanishing Testes Syndrome)

Causes of congenital anorchia 

1)​Fetal Testicular Vascular Accident: This is the most widely accepted cause. It is thought that a twisting of the spermatic cord (testicular torsion) or another vascular event cuts off the blood supply to the developing testes. Without a blood supply, the testicular tissue deteriorates and is absorbed by the body. This is also known as ischemic necrosis.  

2)Genetic Factors: 

While a specific gene has not been definitively linked to anorchia, it is possible that genetic factors play a role in some cases. However, studies have shown that the SRY gene, which is crucial for male sex determination, is typically normal in individuals with anorchia.  

​Hormonal Imbalances: Disruptions in the hormonal environment during fetal development may also contribute to the failure of testicular survival.  

B) Acquired Anorchia

Causes of acquired anorchia

Acquired anorchia is the loss of the testes after birth. This is a less common cause and is usually a result of:  

1)​Trauma: A severe injury to the scrotum that requires the surgical removal of one or both testes (orchiectomy).  

​Infection: A serious infection that causes irreversible damage to the testes, leading to their surgical removal.  

2)​Cancer: The surgical removal of testes as a treatment for testicular cancer.

​Surgical Complications: In rare instances, anorchia can be a complication of other surgical procedures, such as hernia repair, if the blood supply to the testes is accidentally compromised. 

Anorchia symptoms

1)Failure to start puberty at the correct time: Without testosterone, the individual will not experience the typical pubertal changes of a male.

2)Lack of male secondary sexual characteristics: This is a direct result of low testosterone levels and can include:

No or very little growth of pubic and facial hair.

3)No deepening of the voice.

Lack of increased muscle mass and strength.

4)Absence of a growth spurt.

Infertility: Because the testes are necessary for sperm production, individuals with anorchia are infertile.

Reduced libido and erectile function: Low testosterone levels can impact sex drive and the ability to achieve and maintain an erection.

Anorchia Diagnosis 

1)Physical Examination: A doctor will examine the scrotum and groin area for the presence of testes. In anorchia, the testes are not palpable. It’s crucial to distinguish this from undescended testes, which can sometimes be felt in the abdomen or inguinal canal. 

2)Hormone Testing: Blood tests are a critical part of the diagnosis. The results typically show:

Low or undetectable levels of testosterone: This is the primary male hormone produced by the testes.

Low or undetectable levels of anti-Müllerian hormone (AMH) and inhibin B: These hormones are also produced by the testes. 

3)Imaging Studies: Imaging techniques are used to search for any remnants of testicular tissue in the abdomen or inguinal canal.

Ultrasound: A non-invasive imaging method that can be used to scan the scrotum, groin, and abdomen.

Magnetic Resonance Imaging (MRI): Provides more detailed images and can be helpful in locating any residual tissue that may not be visible on an ultrasound.
